VACATION BIBLE SCHOOL MEDICAL RELEASE FORM
In case of emergency, contact those listed on the registration form. They gave permission to St. Mark’s Lutheran Church to obtain all necessary medical care prescribed by the nearest emergency room. This care may be given under whatever conditions are necessary to preserve life, limb, or well-being of my dependent.

PHOTO RELEASE
Additionally, I give permission for my child to e photographed during activities associated with St. Mark’s Lutheran Church Vacation Bible School. I understand that said photos/videos may be used for the St. Mark’s Lutheran VBS Progra, in-house church use and/or for public information for promotion of the church (i.e. brochures, websites, newspapers, radio and television).
