2024 VBS Child Registration

We are excited to be able to hold VBS this summer at St. Mark's Lutheran Church! Our theme will be Scuba: Diving Into Friendship with God!

Please fill out this form to be a VBS Participant. Individuals ages 3 through completion of 5th grade will be VBS participants. Each participant will need to be registered separately. VBS will be held at St. Mark's Lutheran Church on Tuesday, July 9-Thursday, July 11 from 5:00-8:00 PM. Dinner will be provided by the women of the church if individuals would like to eat at the church before programming begins at 5:25 PM. Dinners will be taco night, Italian night, and grill out night. We will do our best to accommodate food allergies, but do note that our kitchen is not nut or gluten free on a regular basis. Feel free to connect directly with the VBS team regarding any allergy concerns.

VACATION BIBLE SCHOOL MEDICAL RELEASE FORM
In case of emergency, contact those listed on the registration form. They gave permission to St. Mark’s Lutheran Church to obtain all necessary medical care prescribed by the nearest emergency room. This care may be given under whatever conditions are necessary to preserve life, limb, or well-being of my dependent.

PHOTO RELEASE
I give permission for my child to be photographed during activities associated with St. Mark’s Lutheran Church Vacation Bible School. I understand that said photos/videos may be used for the St. Mark’s Lutheran VBS Program, in-house church use and/or for public information for promotion of the church including but not limited to the St. Mark's Lutheran Church website, the St. Mark's Lutheran Church Facebook page, brochures, signs, or publicity documents for this or future VBS programs, and the VBS end-of-camp video.
